Fulcrum Customer Satisfaction Assessment Report (CSAR)   

Introduction: Fulcrum appreciates your ratings and comments as a direct input to our goal of continuously improving our customer’s satisfaction with our products and services. We are committed to administering this report at least annually to gather first-hand information on what we have done well and what should be done better. We will use the feedback to identify areas for improvement and shore up strengths that are important to our customers and our management team.

Directions: On the scale below, please rate your satisfaction with our effort from 1 (very dissatisfied) to 10 (very satisfied). The focus is on our clients and associated customer satisfaction. Please read each question or statement carefully and enter a number from 1 to 10 in the box next to the question. If the question or comment is not applicable to you, enter NA. Your input and comments are critical to our success.

Fulcrum managers want to be responsive to your questions and needs.  How satisfied are you with the level of communications?
Please rate your satisfaction with Fulcrum Contracts Representatives’ responsiveness to your Contracts Manager.
